OFFICE MEMORANDUM

Subject: Fresh Empanelment of Health Care Organizations under CGHS Kanpur.

In pursuance of CGHS Office Memorandum F.No.- 5-34/CGHS/HEC(HQ)/2025 dated 22.12.2025, issued by the Directorate CGHS, the undersigned is directed to convey that, the Health care Organizations (HCOs) listed in Annexure-D are hereby freshly/newly empanelled under CGHS in Kanpur.

The empanelment shall remain valid for a period of three (03) years from the date of signing of the MOA mentioned in the Annexure-D against their name. The empanelment is subject to the terms and conditions stipulated in the Office Memorandum dated 22.12.2025. The empanelled hospitals shall strictly adhere to all the terms and conditions mentioned therein, as well as to the guidelines/instructions issued by CGHS from time to time.

It is further clarified that the Health Care Organizations (HCOs) shall claim NABH/NABL rates only for those investigations/procedures which are duly NABH/NABL accredited.
