Ref: IR Healthcare Policy on Cancer Treatment vide RB letter no. 2025/I & Trans Cell/ Healthcare/2/P dt. 05.05.2025 read with Healthcare Policy instructions vide RB letter no. 2024/1 & Trans. Cell/ Healthcare /P dt. 29.08.2024

Railway Board vide letter under ref. has issued IR Healthcare Policy on Cancer Treatment issued and as continuing efforts to provide most advanced healthcare facility available for complex and critical illness like cancer, IR has entered into an exclusive & comprehensive Agreement with the Tata Memorial Centers (TMCs) – a premier cancer institution recognised by the Gol, Ministry of Health & FW and under the aegis of Gol, Dept of Atomic Energy. Any railway employee, pensioner and dependent family members can avail the benefit of cashless cancer treatment at all the eight (08) TMCs. Basic features of the Healthcare Policy instructions on cancer treatment are at Annex-I.

2. IR’s exclusive & comprehensive Agreement/s with TMCs as below, are available on Railway Board (web-page of Health Directorate) www.indianrailways.gov.in —> Ministry of Railways > Railway Board > About Indian Railways > Railway Board Directorates > Health Directorate > MoUs with Units of TMC for Cancer Treatment: –

  1. Tata Memorial Hospital / Mumbai and ACTREC (Advanced Center for Treatment Research & Education in Cancer) / Navi Mumbai
  2. TMC (Homi Bhabha Cancer Hospital & Research Centre) — Muzaffarpur, Bihar.
  3. TMC (Homi Bhabha Cancer Hospital & Research Centre) — Vishakhapatnam, Andhra Pradesh.
  4. TMC (Homi Bhabha Cancer Hospital & Research Centre) — Mullanpur, New Chandigarh.
  5. TMC (Homi Bhabha Cancer Hospital) — Sangrur, Punjab.
  6. TMC (Mahamana Pandit Madan Mohan Malaviya Cancer Centre) — BHU Campus, Varanasi, UP.
  7. TMC (Dr Bhubaneswar Borooah Cancer Institute) – Guwahati, Assam.

3. In order to preclude errors and ensure that 100% of the cancer referral letters are as per the policy instructions on comprehensive & inclusive cancer treatment issued vide Railway Board letter no. 2025/ I &Trans Cell /Healthcare/ 2/ P dated 05.05.2025, the HMIS process flow for generation of a Cancer Referral is given in the box below:

Process flow for generation of a Cancer Referral: HMIS > OPD Doctor’s Desk > Referral / Cross Consultation > Refer Type (Select) Private Empanelled HCO > Department (Select) ONCOLOGY / CANCER.
note: in the clinical remarks on referral letter issued by Railway Doctor no extraneous / non-medical comments should be recorded and strictly only patient’s clinical / medical condition and related remarks that require attention for imparting referred treatment.

4. Railway Board, Health Directorate (Director /Health Policy & Projects) is the ‘single point of contact’ (SPOC) on healthcare policy and issues regarding HMIS.

5. This issues with the approval of DG (RHS).

Cancer Treatment in all the Tata Memorial Centres (TMCs) in India and Mumbai’s Tata Memorial Hospital & Advanced Center for Treatment Research & Education in Cancer (TMH/ACTREC) No referral / permission / endorsement is required for initial Diagnostics / Tests / Consultation / Investigation at Preventive Oncology Branch or OPD in Tata Memorial Centre Hospital (TMCH) – and the medical reimbursement claim for such treatment at TMCH shall be admissible as per rates stipulated in IR’s MoU with TMCH or on actuals, whichever is lower, and may include cost of registration, consultation, initial Tests / Medication etc. in OPD.
For medically expedient cancer cases needing referral to TMCС /TMH (including ACTREC) on the basis of OPD/ Consultations / Investigations / Diagnostics / Tests the cancer patient may exercise the option of either taking referral from jurisdictional RH (mentioned in patient’s UMID)

or take referral in Mumbai itself from any of IR’s two nodal ZRHs for TMCН. namely Dr Babasaheb Ambedkar Memorial Rly Hospital or Babu Jagjivan Ram Rly Hospital.

Referral by Railway Doctor is inclusive of the in-house treatment (Consultation, Investigations, Chemotherapy, Radiotherapy, Surgery etc) During IPD treatment, the robotic / laparoscopic / implants / special surgery etc. as allowed under CGHS and including chemotherapy administered, radiation, medicines dispensed, Tests or other diagnostics done, would be covered in the expenses met during the treatment. Medicines, including post-operative protocol medicines and follow-up medicines, as prescribed shall be provided by the TMCH.

In the event medicines are not provided by TMCH it will certify that the medicine was not provided to the patient and the same would be provided by any RH/HU as per patient’s request.

Cancer treatment in Government Hospitals (GHs) and Institutes of National Importance (INIs) No referral/permission / endorsement is required for cancer treatment including OPD/IPD/Tests in INIs. Reimbursement as per actuals or city specific CGHS rates, whichever lower & may include cost of registration, consultation, initial treatment, medication etc in OPD
For cancer cases needing referral treatment /IPD on the basis of OPD /consultations /investigations /diagnostics /tests, the referral would be given by the jurisdictional RH as per patient’s UMID.
Medicines (post-operative & follow-up) to be provided by the INI If INI or GH – where treatment has been taken – certify that the medicines could not be provided, the same would be provided by any RH/HU as per patient’s request – since UMID has been granted pan-IR validity.
  Referral/permission/endorsement not required for cancer OPD/ Consultations / Investigations / Diagnostics / Tests (except IPD) in empanelled HCOs of IR/C BUT the reimbursement in these cases would be admissible only, and only if cancer /malignancy mangnaney is confirmed in which case is coniirmed in which case reimbursement would be as per actuals or city specific CGHS rates, whichever lower & may include cost of registration, consultation, initial treatment/medication etc in OPD.
  Referral would be given by Jurisdictional RH as per patient’s as pa UMID for cancer cases needing referral treatment / IPD on the basis of OPD / consultations/ investigations / diagnostics/Tests. Referral /treatment to be inclusive. IPD treatment to be as per MoU with respective EH, and chemotherapy administered, radiation, medicines dispensed, Tests & other diagnostics done would be covered in the expenses met during the treatment.
Cancer treatment in any IR Empanelled or a CGHS Empanelled HCO (E) Medicines that the EH could not provide and all postoperative protocol /medicines, as prescribed shall be provided by any RH /HU as per patient’s request.
