No. 5-34/CGHS/HEC/HQ/2025 dated 22nd December 2025, Vivacity Multi specialty Hospital (A unit of Nephro Care India Limited) Holding No. 361/23, Jessore Road (South), Kolkata, PO & PS – Madhyamgram, North 24 Pargana, 700129 is empaneled under CGHS Kolkata with effect from 17th September 2026 for a period of 3 (three) years as per the Terms & Conditions of empanelment laid down in the Memorandum of Agreement.
