Health problems of personnels of para military forces

Details of the various health problems being faced by the para-military forces personnels, which are on the increase:-
Various health problems are faced by Central Armed Police Forces (CAPF) personnel depending upon their nature of duties, place of posting, age and personnel problems etc. As reported by CAPFs & ARs, some of the most common health problems being faced by the CAPF personnel are as under:-

(i) Cardiovascular disease e.g. hypertension, cornory artirary disease,
(ii) Diabetes
(iii) Gastritis, PUS etc.
(iv) Training/work related orthopedic conditions e.g. ACL tears, fractures etc.
(v) Tropical disease e.g. malaria, viral hepatitis, enteric fever.

The reasons for the para-military forces personnels affected by these health problems:-

Most of these are common ailments in the present day society due to life style and dietary habits, but some more specific reasons for the CAPF personnel getting affected by these health problems are as under:-

(i) Nature of duties/occupation as well as training exercises leading to injuries like training, exercise-LRP, blast injury, gun-shot injury, etc.
(ii) Stress due to deployment in difficult terrains and long separation from family.
(iii) Posting in areas having difficult/adverse climatic conditions such as high altitude.
(iv) Personal health problems. 

The steps taken by Government to keep the personnels of para-military forces healthy:-

Various steps are taken by MHA to ensure sound health of the CAPF personnel. Some of the important measures taken in this regard are:

(i) Each CAPFs & ARs (except CISF) has its own medical cadre having sufficient number of Medical Officers, Specialists, paramedical staffs, medical infrastructures etc. to cater to the medical needs of the CAPF personnel and their family members;

(ii) 401 Nos. of 10-bedded Unit hospitals, 32 Nos. of 50 bedded and 06 Nos. of 100 bedded Composite Hospital have been established to provide secondary level medical care to CAPF personnel and their family members. In addition to the above, there are 195 Mobile Hospitals to take care of medical requirements of the Force personnel;

(iii) Regular health screening of all CAPF personnel is done through mandatory Annual Medical Examination. Identification of the risk group and the diseases in its early stage is done meticulously in the Annual Medical Examination for all Force personnel;

(iv) Periodical follow-up of chronic cases (disease) is done regularly;

(v) Troops are periodically advised by way of issuance of Standard Operating Procedures (SOPs) and advisories to create awareness amongst them on various health issues and regarding prevention of various diseases; 

(vi) Medical Officers regularly visit Border Out Posts (BOPs). First Aid Box with adequate medicines and Nursing Assistant are available at all BOPs;

(vii) HIV/AIDS control programme has been implemented in all the CAPFs & ARs;

(viii) Guidelines on diet and sample menu have been circulated to all formations for preventing Hypertension and heart ailments;

(ix) Regular physical activities in the form of morning PT and evening games are being followed in order to keep the personnel fit;

(x) Any disease detected is properly addressed, and if required, the cases are referred to specialists in well equipped hospitals for detailed investigation, diagnosis, opinion and treatment etc;

(xi) Record of each person’s health is kept in his personal health card and reviewed from time to time by the concerned Authorized Medical Attendant and Officer Commanding of the respective Force;

(xii) Workshops regarding awareness on Cardiac and Hypertension related diseases are organized;

(xiii) Multispecialty/Super Specialty hospitals recognized by CGHS have been empanelled in major cities to provide super specialized/specialized treatment for the personnel;

(xiv) Vaccination/Immunization programmes are conducted from time to time as preventive measures; 

(xv) Proper First Aid Training has been imparted to the Force personnel to ensure timely medical attention in case of an emergency;

(xvi) Arrangements have been made by the Forces to provide anti-malarial kits to men who work/belong to endemic areas while proceeding on leave/duty;

(xvii) Stress Management Capsule courses are regularly being conducted for the troops to mitigate their stress level;

(xviii) Provision of recreational facilities and introduction of Regimental and Community activities including Sports/Games, etc. at all levels to ensure maximum participation in order to reduce the stress levels;

(xix) Organizing talks by Doctors and other Specialists with the CAPF Jawans to address their personal/psychological concerns, etc.

(क) : केन्द्रीय सशस्त्र पुलिस बल स्वास्थ्य संबंधी विभिन्न समस्याओं का सामना कर रहे हैं, जो उनकी ड्यूटियों की प्रकृति, तैनाती के स्थान, आयु एवं व्यक्तिगत समस्याओं आदि पर निर्भर करती हैं । जैसाकि केन्द्रीय सशस्त्र पुलिस बलों एवं असम राइफल्स द्वारा सूचित किया गया है, केन्द्रीय सशस्त्र पुलिस द्वारा सामना की जा रही स्वास्थ्य संबंधी सर्वाधिक सामान्य समस्याएं निम्नलिखित हैं :-
(i) हृदवाहिका रोग अर्थात उच्च रक्तचाप, रक्तवाहिनी धमनी संबंधी रोग,
(ii) मधुमेह,
(iii) आमाशय शोध, पी यू एस आदि,
(iv) प्रशिक्षण/कार्य से संबंधित अस्थि रोग की स्थितियाँ अर्थात ए सी एल टीयर्स, फ्रैक्चर्स आदि ।
(v) ट्रॉपिकल रोग अर्थात मलेरिया, वायरल हेपेटाइटिस, आन्त्र बुखार ।

(ख) : इनमें से अधिकतर रोग, जीवन शैली एवं खान-पान की आदतों के कारण आज के समाज में सामान्य बीमारियां हैं, किंतु केन्द्रीय सशस्त्र पुलिस बल के कार्मिकों के इन स्वास्थ्य समस्याओं से प्रभावित होने के कुछ और विशिष्ट कारण निम्नलिखित हैं :-

(i) ड्यूटियों/वृत्ति और प्रशिक्षण कार्य की प्रकृति, जिससे प्रशिक्षण अभ्यास-एल आर पी, ब्लास्ट इन्जरी, गन-शॉट इन्ज्यूरी आदि जैसी इन्जरी होती हैं ।
(ii) कठिन क्षेत्रों में तैनाती एवं परिवार से लंबे समय तक अलग रहने के कारण तनाव ।

(iii) कठिन/प्रतिकूल जलवायु स्थितियों वाले क्षेत्रों जैसे कि अत्यधिक ऊँचाई वाले क्षेत्रों में तैनाती ।
(iv) व्यक्तिगत स्वास्थ्य समस्याएँ ।

(ग) : गृह मंत्रालय द्वारा केन्द्रीय सशस्त्र पुलिस बल कार्मिकों का अच्छा स्वास्थ्य सुनिश्चित करने के लिए विभिन्न कदम उठाए गए हैं । इस संबंध में किए गए कुछ महत्वपूर्ण उपाय निम्नलिखित हैं :-

(i) प्रत्येक केन्द्रीय सशस्त्र पुलिस बल एवं असम राइफल्स (केन्द्रीय औद्योगिक सुरक्षा बल को छोड़कर) का अपना चिकित्सा संवर्ग है जिसमें केन्द्रीय सशस्त्र पुलिस के कार्मिकों एवं उनके परिवार के सदस्यों की चिकित्सा जरुरतों को पूरा करने के लिए पर्याप्त संख्या में चिकित्सा अधिकारी, विशेषज्ञ, पराचिकित्सा स्टाफ, चिकित्सा संबंधी अवसंरचना आदि उपलब्ध हैं;
(ii) केन्द्रीय सशस्त्र पुलिस के कार्मिकों एवं उनके परिवार के सदस्यों को गौण स्तर की चिकित्सा परिचर्या प्रदान करने के लिए 10 बिस्तर वाले 401 यूनिट अस्पतालों, 50 बिस्तर वाले 32 एवं 100 बिस्तर वाले 6 कम्पोजिट अस्पतालों की स्थापना की गई है । उपर्युक्त के अतिरिक्त, बल के कार्मिकों की चिकित्सा संबंधी आवश्यकताओं को पूरा करने के लिए 195 चलते-फिरते अस्पताल हैं;
(iii) अनिवार्य वार्षिक चिकित्सा जाँच के जरिए केन्द्रीय सशस्त्र पुलिस बल के सभी कार्मिकों की नियमित स्वास्थ्य जाँच । बल के सभी कार्मिकों के लिए वार्षिक चिकित्सा जाँच में शुरु में ही जोखिम समूह एवं रोगों का सावधानीपूर्वक पता लगाया जाता है;
(iv) चिरकालिक (क्रानिक) मामलों (रोगों) का आवधिक रुप से अनुवर्ती उपचार नियमित रुप से किया जाता है;
(v) विभिन्न स्वास्थ्य समस्याओं और विभिन्न रोगों की रोकथाम के बारे में उनके बीच जागरुकता पैदा करने के लिए मानक प्रचालन प्रकियाएं एवं हिदायतें जारी करके टुकड़ियों को आवधिक रुप से सलाह दी जाती है;
(vi) चिकित्सा अधिकारी नियमित रुप से सीमा चौकियों (बी ओ पी) का दौरा करते हैं । पर्याप्त दवाओं वाली प्राथमिक उपचार पेटी और नर्सिंग सहायक सभी सीमा चौकियों में उपलब्ध है;
(vii) सभी केन्द्रीय सशस्त्र पुलिस बलों एवं असम राइफल्स में एच आई वी/एड्स नियंत्रण कार्यक्रम कार्यान्वित किया गया है;
(viii) उच्च रक्तचाप एवं हृदय रोगों के निवारण के लिए खान-पान संबंधी दिशानिर्देश एवं सैम्पल मेनू सभी बलों को परिचालित किए गए हैं;
(ix) कार्मिकों को चुस्त दुरुस्त रखने के लिए सुबह के समय पी टी एवं शाम के समय खेल-कूद के रुप में नियमित शारीरिक कार्यकलाप किए जा रहे हैं;

(x) पता लगाए गए किसी भी रोग का समुचित उपचार किया जाता है और यदि आवश्यक होता है तो रोगियों को विस्तृत जाँच, निदान, चिकित्सा संबंधी राय एवं उपचार आदि के लिए सुसज्जित अस्पतालों के विशेषज्ञों को रेफर किया जाता है;
(xi) प्रत्येक व्यक्ति के स्वास्थ्य का रिकार्ड उसके व्यक्तिगत स्वास्थ्य कार्ड में रखा जाता है और संबंधित प्राधिकृत चिकित्सा परिचर और संबंधित बल के कमांडिंग अधिकारी द्वारा इसकी समय-समय पर समीक्षा की जाती है;
(xii) हृदय एवं उच्च रक्तचाप संबंधी बीमारियों के बारे में जागरुकता पैदा करने के लिए कार्यशालाएं आयोजित की जाती हैं;
(xiii) कार्मिकों को अति विशिष्ट/विशिष्ट उपचार मुहैया कराने के लिए बड़े शहरों में केन्द्रीय सरकार स्वास्थ्य योजना (सी जी एच एस) द्वारा मान्यता प्राप्त बहु विशेषज्ञता/अतिविशेषज्ञता वाले अस्पतालों को पैनल में शामिल किया गया है;
(xiv) निवारक उपायों के रुप में टीकाकरण/रोग प्रतिरक्षण कार्यक्रम समय-समय पर आयोजित किए जाते हैं;
(xv) आपातकालीन स्थिति में समय पर चिकित्सा सुविधा प्रदान करने के लिए बल के कार्मिकों को प्राथमिक उपचार संबंधी समुचित प्रशिक्षण प्रदान किया गया है;
(xvi) स्थानिक/भारी क्षेत्रों में कार्य करने वाले/वहाँ रहने वाले लोगों को छुट्टी/ड्यूटी पर जाने के समय मलेरियारोधी किटें प्रदान करने के लिए बलों द्वारा व्यवस्था की गई है;
(xvii) टुकड़ियों का तनाव स्तर कम करने के लिए टुकड़ियों हेतु नियमित रुप से तनाव प्रबंधन कैपसूल पाठ्यक्रम आयोजित किए जा रहे हैं;
(xviii) तनाव स्तर को कम करने के लिए अधिकतम भागीदारी सुनिश्चित करने हेतु सभी स्तरों पर खेल-कूद आदि सहित मनोरंजन सुविधाओं की व्यवस्था करना और रेजिमेंटल एवं सामुदायिक कार्यकलापों को शुरु करना;
(xix) सीमा सुरक्षा बल के जवानों की व्यक्तिगत/मनोवैज्ञानिक चिंताओं को दूर करने के लिए डाक्टरों एवं अन्य विशेषज्ञ चिकित्सों द्वारा उनके साथ बातचीत करना ।
