CGHS Wellness Centre TR III, Vanchiyoor will start functioning in the new building PS Towers, Tutors Lane, Statue, GPO, Thiruvananthapuram 695 001 from 10.07.2023 (Monday).

Please fwd list of required documents for obtaining CGHS facilities to a Pensioner.
1) Retirement letter from the previous served office/PENSIONER CARD.
2) Last Pay Certificate.
3) Details of family members with age from the served office.
4) No objection certificate from served office for not taking F.M.A.
5) 12 months or 120 months subscription as per the pay level amount of
D.D. in favor of CGHS AUTHORITY ZONE.
