Subject: Empanelment of HCOs under Continuous Empanelment Scheme under CGHS, Chandigarh (HQ) – regarding.
In continuation of this office memorandum dated 17.11.2014, even No. dated 04.03.2016, 09.11.2016, 07.02.17, 17.04.17, 17.08.17, 15.11.17, 27.12.17, 03.04.18, 29.06.18, 01.10.18, 27.12.18, 28.03.19, 26.06.19, 24.09, 01.01.2020, 29.06.2020 & 26.03.2021 and in terms of GOI, MOHFW, Dte.Genl.of CGHS (HEC) Nirman Bhawan, New Delhi’s letter No. S.11045/36/2016-CGHS (HEC) dated 01.10.2020, the following HCO is empanelled under CGHS w.e.f 09.06.2021 till next extension or new empanelment whichever is earlier.
