Sub: Modification/Revision of Annexure-VII (Declaration) to Chapter 5 of IRMM 2000 for Medical Examination of Railway Recruitment Candidates-reg.

The matter regarding obtaining a declaration from candidates appointed through direct recruitment has been examined in Board’s Office. Accordingly, certain modifications have been made to Annexure-VII in pursuance of the instructions issued regarding LASIK Surgery and the changes introduced in the recruitment process of Assistant Loco Pilots (ALPs) under CEN No. 01/2026.

Further, it is hereby clarified that these instructions shall be applicable to all future recruitments and medical examinations of candidates.

An Advance Correction Slip (ACS) to Annexure-VII of Chapter 5 of IRMM 2000 is enclosed herewith for necessary action.

1. (a) Have you ever had intermittent or any other prolonged fever, enlargement or suppuration of glands, spitting of blood, asthma, heart disease, lung disease, fainting attacks, rheumatism, appendicitis, ? …or

(b) Any other disease or accident requiring confinement to bed and prolonged medical or surgical treatment/hospitalization? ………..

2. Have you or any of your near relation been afflicted with Tuberculosis, Scrofula, gout, asthma, fits, epilepsy or insanity? ………..

3. Have you suffered from any form of nervousness due to overwork or any other cause?  ………..

4. Have you been examined and declared unfit for Government service by a Medical Officer/Medical Board within the last three years?  ………..

5. Did you ever sustain head Injury? If yes, give detailed history.

6. Do you have fits or epilepsy, attacks of giddiness, vertigo or mental abnormality?

7. Are you in the habit of taking any drugs/alcohol?

8. Do you have Intra Ocular Lens/Contact Lens/history of surgery for correction of eye sight?

9. Are you taking any drug/treatment for M.I./Hypertension/Diabetes Mellitus or any other disease?

10. Do you have any speech or communication disorder, including stammering/stuttering? If yes, give detailed history and treatment details, if any.
